Bugclients stays in server after DC or leave

hello
i have tested a few verseion. from 2.5.1 to 2.5.7
there is a bug in these servers
after a while like 12 hours or more server gets into a bug which clients get invalid response from server
their channel and servergrou wont load instead showing [client-server-group][clietn-channel-group] and so..
and they do not have any permission.its like server wont responde to clients in anyway
also they wont disconnect from server the old one stays in server and server player counts increasing like hell because of it

in debug log i see these logs when the server gots into this bug state
